Please be sure to fill out the form as completely and accurately as possible so as to ensure the quickest handling of your request. You are responsible for the return shipping charges. If an item is returned for repair, we will ship the item back to you at our cost. (domestic US shipments only, international customers are required to pay for their return shipping)
